In accordance with the requirements of our Quality Management System, we are pleased to send you our Annual Satisfaction Survey, which aims to determine the level of satisfaction that STRATESYS customers, in this case << Company >> , have of our professional services, especially during the past 12 months (2015 approx.).

The questionnaire is very simple and is divided into different blocks of questions, each of which refers to different aspects of interest to us. Please rate each question from 1 to 5, with 1 being 'low / bad / dissatisfied' and 5 'high / good / happy'. Furthermore, please feel free to add any comment deemed appropriate in the areas designated for this purpose. Moreover, and also if deemed appropriate, please forward this e-mail to those people in your organization that you think might contribute to the assessment of our services.
